We built six country stores that way for a fashion brand headquartered here, and the decision that matters is agreeing early what is shared and what is local.
Usually a lot, and usually without buying anything. Search runs on Elasticsearch or OpenSearch, and most bad results trace back to attribute weighting, missing synonyms and no rules for the queries that actually convert. When that fashion portal dropped Doofinder we moved it onto Magento default elastic search and tuned it there, which is generally the cheaper answer.
Rarely the platform on its own. It is normally full page cache misses, an unindexed catalogue, a third-party extension doing database work on every request, or images nobody has looked at since launch. We profile before recommending, because replatforming to solve a caching problem is an expensive way to solve a caching problem.
They survive if they were written against Magento extension points rather than on top of core. The audit that opens a maintenance engagement grades each module keep, refactor or retire, and anything we write goes in the same way, so the code you own never becomes the reason you cannot upgrade.
